Short-Term (In-State) Patient Licenses
Oklahoma residents may apply for a Short-Term Medical Marijuana Patient License if their medical need is temporary and does not require a full two-year license.
Short-term, in-state licenses are valid for 60 days from the date of approval.
Short-Term Adult Patient (18+)
Oklahoma residents age 18 and older must provide:
- Valid proof of identity
- Proof of Oklahoma residency
- A compliant passport-style photo
- A completed and signed Physician Recommendation Form (Adult Patient) from an authorized Oklahoma physician
Short-Term Minor Patient (Under 18)
Residents under age 18 applying for a short-term license must meet additional requirements:
Two physician signatures on the Physician Recommendation Form (Minor Patient)
A parent or legal guardian must:
- Provide proof of identity
- Provide proof of Oklahoma residency
- Submit an acceptable photo
- Sign the application
Short-term minor licenses are subject to the same regulations and restrictions outlined in the Minor Patient License section.
Do I Need a Caregiver?
If a patient requires assistance managing medical marijuana, a licensed caregiver may be designated.
- The recommending physician must indicate on the Physician Authorization Form that a caregiver is required.
- The patient and caregiver must complete and sign a Caregiver Designation Form.
Application Fees
The state application fees are nonrefundable and paid directly to the Oklahoma Medical Marijuana Authority (OMMA):
$100 application fee + $4.30 processing fee
Reduced fee of $20 + $2.50 processing fee for applicants who provide proof of:
- Enrollment in Medicaid (SoonerCare)
- Medicare
- 100% Disabled Veteran status
