Caregiver Licenses
A Caregiver License is available to a family member or trusted assistant who provides regular support to a licensed medical marijuana patient. This license authorizes the caregiver to legally purchase, transport, possess, and administer medical marijuana or medical marijuana products on behalf of their designated patient(s).
Who Can Be a Caregiver?
A caregiver may be designated to assist patients who require help managing their medical marijuana use.
- Each licensed caregiver may serve up to five (5) licensed patients.
- Licensed adult patients may designate one (1) caregiver.
- Licensed minor patients may designate up to two (2) caregivers.
What Is Required?
To establish a caregiver relationship:
- The patient must have a completed Physician Recommendation Form (Adult or Minor), including the section where the physician certifies that a caregiver is necessary.
- Both the patient and caregiver must complete and sign a Caregiver Designation Form.
License Duration
Caregiver licenses remain valid until the expiration date of the associated patient’s license, not to exceed two (2) years.
There is no state application fee for caregiver licenses.
Application Requirements
Caregiver applicants must submit:
- Valid proof of identity
- Proof of Oklahoma residency
- A compliant passport-style photo
How to Add a Caregiver (Step-by-Step)
1. Confirm a caregiver is needed
Talk with the patient’s recommending physician to confirm a caregiver is medically necessary.
2. Complete the Physician Recommendation Form
The patient must have a completed Physician Recommendation Form (Adult Patient or Minor Patient), and the physician must fill out the section certifying that a caregiver is required.
3. Choose the caregiver
Select a trusted family member or assistant who will regularly help the patient.
4. Gather caregiver application items
The caregiver will need:
- Proof of identity
- Proof of Oklahoma residency
- A compliant passport-style photo
5. Complete the Caregiver Designation Form
The patient and caregiver must both sign the Caregiver Designation Form to officially establish the relationship.
6. Submit the caregiver application
Submit the caregiver application with the required documents and forms through the state’s licensing portal.
7. Wait for approval before purchasing for the patient
The caregiver should only purchase, transport, possess, or administer medical marijuana for the patient after the caregiver license is approved.
8. Know the limits
- Adult patients may have one designated caregiver
- Minor patients may have up to two designated caregivers
- Caregivers may assist up to five licensed patients
