Adult Patient Licenses

Oklahoma residents who are 18 years of age or older may apply for a Medical Marijuana Adult Patient License with the following:

• Valid proof of identity
• Proof of Oklahoma residency
• A compliant passport-style photo
• A completed and signed Physician Recommendation Form (Adult Patient) from an authorized Oklahoma physician

Once approved, adult patient licenses are valid for two (2) years.

Do I Need a Caregiver?

If a patient requires assistance managing their medical marijuana, a licensed caregiver may be designated. To do so:

• The recommending physician must indicate on the Physician Authorization Form that a caregiver is required.
• The patient and caregiver must complete and sign a Caregiver Designation Form.

Application Fees

The state application fees are nonrefundable and paid directly to the Oklahoma Medical Marijuana Authority (OMMA):

$100 application fee + $4.30 processing fee

Reduced fee of $20 + $2.50 processing fee for applicants who provide proof of:

• Enrollment in Medicaid (SoonerSelect)
• Medicare
• 100% Disabled Veteran status
