New Patient Cards
New patients must establish a Bonafide doctor/patient relationship with our certifying physicians. Doctors at Alternative Wellness will review your medical history, including your medical records and medications during the consultation. You must have one or more Arkansas Approved Qualifying Conditions.
*State law prohibits members of the Arkansas National Guard and the United States Military from obtaining a registry ID card.

The Application Process
The online application process begins at the Arkansas Department of Health website.
You will need:
- Arkansas State ID (Arkansas driver’s license or Arkansas state ID issued by DMV)
- Physician Written Certification (only the ADH form)
- $50.00 debit or credit card payment (American Express not accepted)
Registry ID card application fees are NON-REFUNDABLE.
Cards will be issued within 14 Days from the date of receipt.
When does the physician certification expire?
The physician certification is valid for 30 days from the date the doctor signs the form. If a patient gets a certification and fails to submit it to the ADH within 30 days, they must get a new certification.
State Approval
Once your application has been approved, the Arkansas Department of Health will mail you a registry identification card. Your Medical Marijuana Card serves as your legal protection for medical cannabis use. State law permits patients to purchase up to 2.5 ounces of medical cannabis every 14 days from a licensed Arkansas dispensary.
