How to Apply for Your Oklahoma Medical Marijuana Card

Once you have completed your telehealth consultation with Alternative Wellness, you have 30 days to submit your recommendation to the state. Follow these 5 steps to complete your application.

1. Gather Your Documents

Before you start the online application, make sure you have digital copies (photos or scans) of the following:

  • Your Alternative Wellness Physician Recommendation: (The form we just provided you).
  • Proof of Identity: (Front and back of your OK Driver’s License, State ID, or U.S. Passport).
  • Proof of Residency: (Your OK ID usually covers this, or a utility bill from the last 30 days).
  • A Patient Photo: A clear, color, full-face photo against a plain white or off-white background (think "passport style").
  • Reduced Fee Proof (If applicable): Your Medicare/Medicaid (SoonerCare) card or a VA benefit letter showing 100% disability.

2. Create Your OMMA Account

Visit the OMMA MedPortal and click "Access Portal."

  • Select "Sign Up Now" to create an account.
  • You will receive a verification email. Click the link in that email to activate your account.

3. Start a "New Patient Application"

Log in and select "Apply for a New License" and then "Patient Portal."

  • Physician Information: You will need to enter the details from your recommendation form. Ensure you enter the doctor’s name and board certification exactly as it appears on the form to avoid rejection.
  • Physician Registry: Our doctors are fully registered with the OMMA as required by SB 1066. If the portal asks, you can confirm they are on the state’s 2026 registered list.

4. Pay the State Fee

The state requires a separate application fee paid by credit or debit card:

  • Standard Fee: $100 (plus a small processing fee).
  • Reduced Fee: $20 (for Medicare, Medicaid/SoonerCare, or 100% Disabled Veterans).

5. Check Your Mailbox

OMMA has 14 business days to review your application.

  • Approval Email: You will receive an email as soon as you are approved.
  • Wait for the Card: Your physical card will arrive via USPS.
  • Pro Tip: As of 2026, you can use your OMMA Approval Email as a valid ID at many dispensaries while you wait for your physical card to arrive!

The 30-Day Clock

Your recommendation is valid for 30 days from today. Oklahoma law is strict: if you submit your application on day 31, it will be rejected, and you will need a new consultation. We recommend finishing your state application within the next 48 hours while the details are fresh.

Checklist for a Successful Application

To ensure the OMMA approves your application the first time, please double-check these requirements:

  • Physician Registry (SB 1066): Our physicians are fully registered with the OMMA as required by the new 2026 laws. When the portal asks for your doctor’s info, use the exact details on your attached form.
  • The Photo: Use a clear, color photo against a plain white background. No glasses, no hats, and no "filters."
  • The ID: Ensure your Oklahoma ID photo is bright and clear with no camera flash glare over your name or birthdate.
  • Fees: Have your payment card ready. The state fee is $100 ($20 for Medicaid/Medicare/100% Disabled Veterans with proof).

Good News: No Waiting for the Mail!

As of January 2026, once the OMMA sends your Approval Email, you don't have to wait for the plastic card to arrive in the mail. You can print that email (or show it on your phone) along with your photo ID at most Oklahoma dispensaries to make your first purchase immediately.
